Help with common requests
Use our online services to request many of the things you need from the practice without booking an appointment.
Register with the practice
Update your details
Tell us if you've changed:
Address
Telephone number
Email address
Name
Date of birth
If you've changed your name or date of birth, you'll need to provide supporting documentation.
If you've moved home, you'll also need to live within our practice boundary and provide proof of your new address.
Request a fit (sick) note
If you've been off work for more than 7 days, you can request a fit note online.
Please allow up to 2 working days for your request to be processed.
If you've been off work for 7 days or less, you do not need a fit note. Your employer may ask you to complete a self-certification form instead.
Referrals
I've been referred - what happens next?
Once your GP has referred you, the hospital or specialist service will review the referral before offering an appointment. Sometimes they may redirect it to a more appropriate service.
Different departments use different referral systems, so waiting times and tracking methods vary.
Waiting for an appointment?
You can:
Check your NHS App (where available)
Contact the hospital or clinic directly
Check expected waiting times on My Planned Care
If you don't know where you've been referred, submit a Referral follow-up request through our online service.
Need a private referral?
Request a private referral letter through our online service. If your insurer requires us to complete additional paperwork, this is non-NHS work and charges may apply.
Test results
The quickest way to view your results is through the NHS App.
The app lets you:
View your GP and many hospital test results
Read your GP's comments
Compare previous results
If you cannot access the NHS App, request your results using our online service.
Find a pharmacy
Community pharmacists can help with many common illnesses and provide advice without you needing to see a GP.
Feedback
We are always keen for feedback - positive or constructive. This helps us to constantly improve the services that we offer. We particularly appreciate receiving positive feedback via Google.
If you have a suggestion on how we might improve our services, please complete an online form with as much detail as possible.
Complaints
If you are unhappy about the treatment and care you have received and would like to complain, this can be done verbally or in writing. If you would like to speak to one of our operations managers - Anna and Sarh, this can be booked as an appointment depending on their availability.
You can complain in writing by sending a letter in the post, via reception, or via the website. Our Complaints process leaflet contains further information.
